Contact the Dialysis Technology Department or the Allied Health office for advisement. Obtain your Application for clinical the first day of class, and complete it by the 10th day of classes. Complete the criminal background included in the application form by the 7th day of class. Complete the Health form by 30th day of class. Participate in didactic, laboratory, and clinical practice as scheduled.
