General Admissions Process (For University)
- Have you formally applied to the University by completing the Online Admission Application?
- Did you declare Polysomnographic Technology as your Major of Study?
- Have you paid the Admissions Application Fee online when submitting the web application, by mail with a cashier’s check or money order, or by credit card by calling the Cashier at 318-670-9305?
- Have you submitted evidence of immunity against vaccine-preventable diseases to include but not be limited to measles, mumps, rubella, tetanus/diphtheria, and meningitis- or completed the Immunization Waiver Form?
- Have you uploaded your documents and waivers to the Office of Admissions Secure Document Upload Link?
- Have you completed the Free Application for Federal Student Aid (FAFSA)?
- SUSLA FAFSA Code: 007686.
- Have you scheduled and taken your Accuplacer Placement Assessment for reading, English, and or math?
Admissions Process (For Program)
- Have you spoken with the Program Director about your interest in the program?
- Have you completed the Pre-Requisite Courses? (If not, do so before proceeding)
- SENL 101S: Freshman English I
- SBIO 221S: Human Anatomy & Physiology Lecture I
- BIOL 222S: Human Anatomy & Physiology Lecture II
- ALLH 210S: Medical Terminology
- MATH 133 or Higher (*College Algebra)
- Have you taken the HESI Exam?
- Did you send a copy of your HESI results to the Program Director?
- Have you completed the Polysomnographic Technology Program Application?
- Have you paid the Program Application Fee by mail with a cashier’s check or money order or by credit card by calling the Cashier at 318-670-9305?
- Did you send proof of payment to the Program Director?
- Have you responded to the Application Review Email indicating your intent to attend?

You are now on your way to earning an Associate’s Degree in Polysomnographic Technology. The next step is to complete the remaining courses for this degree with a “C” letter grade or higher. Contact the Program Director for assistance with navigating this process.
Transfer Students Only (If you want to transfer credit from other colleges to SUSLA)
- If you want to transfer college credit to SUSLA, have you sent a copy of your official transcripts to admissions@susla.edu?
- Have you submitted a formal copy of your RPSGT Certificate to the program director?
- Have you had an Academic Degree Plan completed by an Advisor at SUSLA?
- Have you completed the Transfer Articulation Request Form?
- Have you uploaded both the Signed Academic Degree Plan & Transfer Credit Articulation Request to the Office of Admissions Secure Document Upload Link?
Credit by Experience Process (Complete AFTER you have 15 Credit Hours at SUSLA)
- Have you completed 15 semester hours of credit at Southern University?
- Have you completed the Request for Credit by Experience form for EACH of the following courses?
- PSGT 120S: Polysomnography I
- PSGT 140S: Respiratory Care Procedures
- PSGT 145S: Polysomnography Clinical I
- PSGT 155S: Polysomnography Clinical II
- PSGT 160S: Polysomnography II
- PSGT 210S: Polysomnography Clinical III
- PSGT 216S: Polysomnographic Technology Capstone
- Did you turn in each Request for Credit by Experience form to the Registrar’s office either by dropping it off, emailing a copy to registrarsoffice@susla.edu, or uploading it the Secure Document Upload Link?
